Mystery solved - sort of

The Play table was a gift from Bubbie Cookie, but we still can't figure out how or why it was shipped with the name Volkan Coklu. The best guess I have is that Mr. Coklu operates an Amazon store by which he resells items he buys at bulk discount at Sam's club.

Still weird, but at least we're not worried about some scam. :)
